Process of Submitting Applications: In the case of tool destroyed in any disaster, Grama Niladhari should be informed in writing.

Fees Payable for Obtaining Application: No fees payable

Time of Which Application Should be Submitted: Immediately after disaster

Fees Payable for Obtaining the Service: No fees is charged

Time Taken to Provide the Service (Ordinary Service and Priority Service): With in 7 days from receipt of recommendation from the Division Secretary

• Letter certifying damage, Issued by Grama Niladhari and Social Service Officer
• Copy of the complained the police
