After Publishing Newspaper Advertisements, Applications can be obtained from Head Office or Relevant District Offices.

Fees Payable to obtain Applications	-	Rs 175.95/- (Including VAT)
Duration for Submission of Application	-	From 9.00 a.m. to 3.00 p.m on office days
Fees Payable to Obtain the Service	-	Nil

Documents Required
National Identity Card or Other Document to prove the Identity.
