The Business organization must be registered.

The applicant should have registration certificate and import? license issued by the Cosmetics Devices and drugs authority.(Ministry of Health). |
Homoeopathic drugs |
The applicant should have the letter of approval issued by the Sri Lanka Homeopathic Council. |
Veterinary drugs |
The applicant should have the letter of approval issued by the Department of Animal Production and Health in Peradeniya. |
Ayurvedic medicine |
The applicant should have the letter of approval issued by the Department of Ayurveda. |
Surgical equipment |
The applicant should have the letter of approval by the Cosmetic Devices and Drugs Authority (Ministry of Health) &(Department of Animal Production and Health). |

Procedure
Step 1: Applicant should obtain indent form from chief clerk of the Unit 01 drugs.
Step 2: Applicant should submit 4 copies of filled indent form and 4 copies of invoice with support documents.
Step 3: Unit 01 checks the indent forms and support documents
Step 4: License request is put to Import/Export controller for approval
Step 5: Assistant controller approves the License request and applicant should pay the license Fee 0.1% of invoice value
Step 6: Licenses are prepared using computer and submitted to Assistant Controller for approval
Step 7: Assistant Controller puts signature.
Step 8: Out of the 3 copies, 2 are given to the applicant and the Bank and the other one is kept with the department.
Note: If the submitted information is incorrect the request will be rejected.

Validity Period for these Items will be 06 months
Costs Related to the Service
Fee : License Fee – 0.2% of the Invoice value
Penalties : If any applicant has imported goods without a license, the Assistant Controller will decide the percentage to be charged on the importation value.
Surcharge : No surcharge involved.
Special Cases
If the Importation is done without a license, the Import License is issued with a penalty. Following documents are needed to obtain the Import License
