PROCEDURE TO OBTAIN CERTIFICATION
� � �-Quality Systems Certification Scheme (ISO 9001) The Quality Systems Certification Scheme is designed as per ISO 17021 to meet international requirements in certifying quality systems of organizations according to ISO 9001:2008 standard. This scheme is accredited for nineteen scope sectors by RvA, The Netherlands, which is one of the world renowned Accreditation Bodies and by the Sri Lanka Accreditation Body (SLAB) for 20 scope sectors. Hence, the certificates issued under this scheme are recognized all over the world. This scheme is operated with the support of a strong in-house resource pool of Auditors who are fully competent, qualified, experienced and registered with IRCA-UK. There are about 15 Lead Auditors and 65 Auditors currently attached to SLSI. Scope Sectors Accredited By RvA Netherlands

POST CERTIFICATION ANNUAL REGISTRATION Certificates with accreditation mark LKR 80 000.00 Certificates without accreditation mark LKR 70 000.00 12%VAT will be charged on all payments. Transport and accomodation for auditors shall be provided by the client. If the client is unable to provide transport a fee of LKR 30.00 per km will be charged for each assignment (The number of days and the assignment of the number of Auditors will be based on the size of the Organization) Concessions For additional certifications 10% for the second and 20% for the third certification onwards EMS certification is a voluntary certification scheme of SLSI which supports the industry to comply with the requirements of the ISO 14001 standard and to demonstrate company's commitment to environmental protection. � ISO 14001 standard does not set requirements for any environmental aspect such as emissions or discharges: instead organizations establish their own targets of control based on current performance data or to comply with regulatory requirements. � This scheme is accredited for 13 scope sectors by RvA, The Netherlands, which is one of the world renowned accreditation bodies and by the Sri Lanka Accreditation Body (SLAB) for 11 scope sectors. A certificate holder develops and maintains its food safety assurance programme based on the internationally accepted principles of ISO 22000 and SLS 1266. Both Schemes are accredited by RvA, The Netherlands, which is one of the world renowned Accreditation Bodies. Thus the certificates issued under this scheme are recognized all over the world HACCP or ISO 22000 certificate awarded from the SLSI implies that chemical, physical and biological hazards encountered during production or processing of the food item, as applicable to the scope of certification are controlled to make the food safe for human consumption. POST CERTIFICATION ANNUAL REGISTRATION SLS 1266:2005 (HACCP) Certificates with accreditation mark LKR 60 000.00 Certificates without accreditation mark LKR 50 000.00 ISO 22000:2005 (FSMS) Certificates with accreditation mark LKR 80 000.00 Certificates without accreditation mark LKR 70 000.00 12%VAT will be charged on all payments. If the client is unable to provide transport a fee of LKR 30.00 per km will be charged for each assignment (The number of days and the assignment of the number of Auditors will be based on the size of the Organization) Concessions For additional certifications 10% for the second and 20% for the third certification onwards OHSAS 18001 Certification Scheme (Occupational health and Safety Management system) This scheme enables an organization to control its occupational health and safety risks and improve its performance by minimizing employee related risks. If the client is unable to provide transport a fee of LKR 30.00 per km will be charged for each assignment (The number of days and the assignment of the number of Auditors will be based on the size of the Organization) SA 8000 Certification scheme (Social Accountability) It is based on the conventions of the International Labour Organization, the Universal Declaration of Human Rights as well as the United Nations Convention on the Rights of a child. APPLICATION FORMS ARE ISSUED FREE OF CHARGE Application fee (Non-refundable) LKR 500.00 Evaluation of GMP Manual LKR 1000.00 Fee for preliminary assessment LKR 2500.00 Fee for additional assessment LKR 2000.00 per assessment Fee for final assessment LKR 3000.00 12%VAT will be charged on all payments. Transport and accomodation for auditors shall be provided by the client. If the client is unable to provide transport a fee of LKR 30.00 per km will be charged for each assignment.
