Required certificates:

• Application form
• Profoma Invoice
• Pedigree certificate of the donor animal
• Country of origin certificate (if the country of export if different)
• Specimen of the International Veterinary Health Certificate issued by the government authority

Staff Officers in charge of the services:

Alternatives or circumstances other than the above requirements and specific information:  Import semen species and  breed Approval from Director /Breeding, DAPH, Sri Lanka
