Obtaining an identity card under the one-day service

1. The issuance of National Identity Cards by one-day service has been started from 01.09.2003 for applicants who wish to obtain National Identity Cards urgently for urgent needs and the National Identity Card can be obtained from the head office of the Department of Registration of Persons at Battaramulla or from the Southern Provincial Office. can.

 

2. Village Officer and Divisional Secretary of the domain where the applicant resides, Estate Superintendents for Estate Residents, Principal or Parivenadhipa Thero of the respective school for school applicants.

 

 

 

 

 

National ID card applications certified by can be forwarded to the One Day Service. Applications certified by the Village Officer must be countersigned by the concerned Divisional Secretary.

 

3. National ID card processing fee by one day service is Rs. 2000/- (in addition to the application fee).

 

4. It is mandatory for the applicant to be present for the one-day service to send the application and to get the national identity cards.

 

 

 

5. Documents required for obtaining National Identity Card

 

1. Completed P.D.V.1,7,8 form form

 

2. If the National Identity Card is amended, the last obtained National Identity Card

 

3. If the National Identity Card is missing, the original copy of the police complaint (not exceeding 06 months)

 

4. ICAO Standard Photograph taken within 6 months

 

5. Receipt of payment of prescribed application fees (first time application for National Identity Card - Rs.200/-, revision of National Identity Card - Rs.500/-, duplicate of National Identity Card - Rs.1000/-)

 

6. Birth certificate or an extract from the birth register

 

7. A document proving name and date of birth along with presumptive age certificate in case of absence of birth certificate (child's birth / school leaving certificate / estate leaving certificate / copy of horoscope certified by Divisional Secretary or authorized officer / marriage certificate / citizenship certificate/ Original copy of affidavit)

 

8. Applicants above 40 years of age without Birth Certificate or Presumptive Age Certificate with proof of their name and date of birth
