iv) Cash credit slip

Being a company registered under the companies act of Sri Lanka

•Duly filled application form – Form C
•Signature Cards (one per each person operating the account)
•Cash Credit Slip
•Resolution of the Board of Directors
•Company Registration Certificate
•Form 48 – Information about Directors and Secretaries (Form 18 & 20 according to the New Companies Act)
•Form 36 – Registered Address of the Company (Form 13 according to the New Companies Act)
•Form 63 – Annual Income (Form 15 according to the New Companies Act)
•Company Seal (Emboss)
•Names of the Board of Directors of the Company
