Eligibility requirements for obtaining a National Identity Card for the first time (a) Being a Sri Lankan citizen. (b) Being a person who has attained the age of 15 years. Obtaining the National Identity Card application form (P.L.D.V.1,7,8 format). By Application Certifying Officers (Village Officer of the domain where the applicant resides/Estate Residents concerned Estate Superintendent/School applicants Principal or Principal of the concerned school) By the Regional Office of Registration of Persons established in the Regional Secretariat By a Provincial Office of the Department of Registration of Persons (Southern Provincial Office, North Western Provincial Office, Eastern Provincial Office or Northern Provincial Office) By the Operations Section of the Registration of Persons Department Head Office at Battaramulla Through the Department website at www.drp.gov.lk Documents to be submitted for obtaining National Identity Card 1. Correctly completed P.L.D.V.1,7,8 application form ICAO standard photograph taken within 6 months Rs. 200/- application fee paid receipt 4. Birth certificate or an extract from the birth register A document proving name and date of birth along with presumptive age certificate in case of absence of birth certificate (child's birth/school leaving certificate/estate leaving certificate/copy of horoscope certified by Divisional Secretary or authorized officer/citizenship certificate/original copy of affidavit) Applicants above 40 years of age without birth certificate or presumptive age certificate to verify their name and date of birth · Document search result note issued by Additional District Registrar · An acceptable written proof to prove the name and date of birth (child's birth certificate / school leaving certificate / estate leaving certificate / copy of horoscope certified by Divisional Secretary or authorized officer / marriage certificate / citizenship certificate / adult identity card) 7. Original copy of formal affidavit stating name, date of birth and place of birth where necessary Novice Certificate or Upasampada Certificate obtained from the Department of Buddhist Affairs if a Buddhist monk For priests of other religions, certificates obtained by the concerned departments verifying the priesthood Certificates obtained from the concerned departments confirming the fact that ordination/priesthood has been abandoned. Marriage certificate if required to obtain husband's name Citizenship certificate issued by the Immigration Department stating that a person born abroad to a Sri Lankan mother or father has acquired Sri Lankan citizenship. Dual Citizenship Certificate if a dual citizen Applicants who have not applied for the National Identity Card within the appropriate prescribed period, stating the reasons for the delay, must be certified by the Village Officer and the Divisional Secretary and submitted to the Commissioner General in Form P.L.D./Me/2/3/ii (the same The form can be obtained from the Departmental website www.drp.gov.lk or from National ID Certification Officers) and the relevant Rs. 2500/- fine paid receipt 9x4 cm blank envelope with stamp of Rs.110/-
