Application for duplicate copy of National Identity Card due to loss of National Identity Card Obtaining the National Identity Card application form (P.L.D.V.1,7,8 form). By Application Certifying Officers (Village Officer of the domain where the applicant resides/Estate Residents concerned Estate Superintendent/School applicants Principal or Principal of the concerned school) By the Regional Office of Registration of Persons established in the Regional Secretariat By a Provincial Office of the Department of Registration of Persons (Southern Provincial Office, North Western Provincial Office, Eastern Provincial Office or Northern Provincial Office) By the Operations Division of the Head Office of Registration of Persons at Battaramulla Through the Department website at www.drp.gov.lk Documents to be submitted for getting duplicate copy of National Identity Card 1. Completed P.L.D.V.1,7,8 form 2. The original copy of the police complaint about misplaced National Identity Card (not more than 06 months old) 3. ICAO Standard Photograph taken within 6 months 4. Rs. 1000/- Application fee paid receipt 5. Birth certificate or an extract from the birth register 6. Document proving name and date of birth along with presumptive age certificate in case of absence of birth certificate (child's birth / school leaving certificate / estate leaving certificate / copy of horoscope certified by Divisional Secretary or authorized officer / citizenship certificate / original of affidavit version ) 7. Applicants above 40 years of age without birth certificate or presumed age certificate to prove their name and date of birth · Document search result note issued by Additional District Registrar · An acceptable written proof to prove the name and date of birth (child's birth certificate / school leaving certificate / estate leaving certificate / copy of horoscope certified by Divisional Secretary or authorized officer / marriage certificate / citizenship certificate / adult identity card) 8. Original copy of formal affidavit stating name, date of birth and place of birth where necessary 9. Novice Certificate or Upasampada Certificate obtained from the Department of Buddhist Affairs if a Buddhist monk. 10. Certificates obtained by the concerned departments for priests of other religions 11. Certificates obtained from the concerned departments confirming the fact that ordination/priesthood has been abandoned. 12. Marriage certificate if required to get husband's name 13. Citizenship certificate issued by the Immigration Department stating that a person born abroad to a Sri Lankan mother or father has acquired Sri Lankan citizenship. 14. Dual citizenship certificate in case of dual citizenship 15. A blank 9x4 cm envelope with stamp of Rs.110/-
Charges A fee of Rs.1000/- will be charged for obtaining a duplicate of the National Identity Card.
Accordingly, the National Identity Card application form should be completed and submitted to the application certifying officers along with the necessary documents.
