In the occasions when a Relief Stamp is lost or missed or damaged

Qualifications

 

  • Should have been entitled to a Relief Stamp (Samurdhi / Nutrition / Kerosene).
  • Such a Stamp being destroyed by flood or fire or.
  • Such a Stamp being subjected to rat biting / fading / tearing off or.
  • Being missed or lost.
Procedure for Submitting Application Forms
  • The relevant Samurdhi Beneficiary should inform about the loss of his Relief Stamp to his division’s Samurdhi Development Officer and the Cooperative Shop where he purchases goods from, and attempt to find out it.
  • He / she should make a complaint to the Police Station about the loss of Relief Stamp too.
  • If such Relief Stamp can not be found, the “B” part of it retained with Cooperative Shop should be taken and sent through Samurdhi Development Officer to the Divisional Secretary with a Police report, an affidavit and a recommendation letter from relevant Samurdhi Development Officer.

The Time consumed in Obtaining Services (Normal Service - Priority Service)
  • Within one month of time after submitting all required documents to the Divisional Secretarythrough relevant Development Officer, a new Relief Stamp which is validated only for coming months will be issued.

  • An appeal from the applicant.
  • “B” part of the Relief Stamp retained with Cooperative Shop that is entitled to the beneficiary.
  • Samurdhi Development Officer’s recommendation letter.
  • An affidavit.
  • A copy of police report.

2. Rat biting / Tearing off / Drenching / Getting dust

  • An appeal from the applicant.
  • “B” part of the Relief Stamp retained with Cooperative Shop that is entitled to the beneficiary.
  • Samurdhi Development Officer’s recommendation letter.
  • Remained part of the damaged Relief Stamp.

Exceptional Cases or External Instances and Special Information to the above Requirements
  • In a situation where residing house and all belongings have been destroyed by flood or fire, in such an instance, the Divisional Secretary at his discretion has the authority to issue a new Relief Stamp which is valid from the month after destruction occurred instead of lost Relief Stamp by retaining the “B” part of it with him.
  • In a situation where an application form has been submitted for a new Relief Stamp by informing that a Samurdhi Relief Stamp becomes worthless due to rat biting, tearing off, drenching or getting dust, on a careful examination of that application and considering the real condition of the Relief Stamp, and justifiable reasons of that application, the Divisional Secretary at his discretion has the authority to issue a new Relief Stamp validating only the stamps for which are in such condition by retaining the “B” part of relevant Relief Stamp with him.
