Qualifications

• Should be a mother living in Samurdhi Beneficiary family registered in the list of mothers maintained by family health planning officer working at rural level under Ministry of Health & Nutrition Care.
• In case of a mother living in a very poor family which does not receive Samurdhi Benefits where as such mother is recommended personally by Divisional Health Officer that she is with more malnutrition (Not ideal weight for ideal height), and also certified by Samurdhi Development Officer that her name is in the firs line of the registration list a eligible criterion for receiving Samurdhi is entitled to have this Nutrition Package.

Procedure for Submitting Application Forms

• Health family planning officer will issue necessary application form for this purpose.
• This application form should be completed by beneficiary and signed by health family planning officer, and submitted to the relevant Samurdhi Development Officer who is working in the particular Grama Niladhari Division.

• This application form can be obtained from the office of the family health planning officer.

The Time consumed in Obtaining Services (Normal Service - Priority Service)

• On the completion of criterions, if applications form duly filled and having been submitted to the office of the Divisional Secretary in any working day, due stamp can be obtained through the relevant Samurdhi Development Officer within two days.

Required Proof Documents

• A copy of the list for particular month certified by Divisional Health Officer which includes mother’s names should be submitted to the Divisional Secretary.
• In case of a mother living in a family which does not receive Samurdhi Benefits, recommendation from Divisional Health Officer that the person who are in very low nutrition is must.
• And also need Certificates from Samurdhi Development Officer and Samurdhi Manager which confirm that the person is eligible to have Samurdhi Benefits.

Exceptional Cases or External Instances and Special Information to the above Requirements

• A Nutrition Relief Card value of Rs.500.00 with 20 stamps will be entitled for 20�monthe from pregnebt to 1 year after child birth. (8 for pregnent monthers and 12 for lactating mothers)
• A Nutrition Package includes the following items for each stamp per month can be purchased from Cooperative Shop / franchised Sales Outlet.

Food Items:

• White / Red rice, Samba rice / boiled rice – 5kg.
• Grams / Kawpee / Green gram / Dhal / Mysoor dhal / Sri Lanka dhal – 500g
• Egg / dried sprats / dried fish / semen (08 eggs or dried fish equal to it)

1. As issuing at least 04 food items to a beneficiary mother out of the above is essential, according to the food items available within Rs.500/-, the amount can be changed.
2. Out of 20 stamps valid for 20 months, 08 and 12 stamps respectively have been allocated to be issued in respect of pre-maternity and post-maternity.
3. However it can be applied for this from the month when registered with family health officer and the stamps for future months will be validated from the date when duly filled application forms were submitted to the office of the Divisional Secretary.

When appeals for receiving relief or for any other matter are referred, such appeals can be sent to the Samurdhi Commissioner General.
