Qualifications
  • The mother should belong to a family which receives Samurdhi benefits.
  • A child is entitled to have the Nutrition Relief for 12 months from the month arises after one month of its birth.
Procedure for Submitting Application Forms
  • A necessary duly filled application form for this purpose should be submitted by Beneficiary to the Samurdhi Development Officer who is appointed for the relevant Grama Niladhari Division within one month of time after the birth of that child.
The Place where the Application Forms can be obtained
  • The form can be obtained from the Samurdhi Development Officer of the relevant Grama Niladhari Division or from the Office of the Divisional Secretary.

  • The application form can be submitted to the relevant Samurdhi Development Officer during the office hour in a working day of a week.

Exceptional Cases or External Instances and Special Information to the above Requirements
  • The application form should be submitted by child’s mother or father.
  • In case of a twins, a report containing mother’s name, relevant Grama Niladhari Division, child’s date of birth and its name recommended by Divisional Secretary should be sent to the Samurdhi Commissioner General in order to issue Samurdhi Relief Stamps for additional child.
  • In a special case, due to an unavoidable reason where a delay occurs in issuing a Nutrition Relief Stamp to a mother, then only instructions will be given to the Office of the Divisional Secretary by Commissioner General of Samurdhi in respect of making Stamps of last month validated.
