01.With customs documents
1.Perfected MTA-2 application Form ( Official stamp should be affixed and signed in relevant places by the agency from which the purchase was made)
2.A certified copy of the National Identity Card or valid Driving licence of valid passport ( should certified by the Grama Niladhari of the Wasam where the applicant is residing/ Commissioner of Oaths/ Attorney law/ justice of Peace)
3.Two (2) certified photograph of the applicant ( size 3.5x2.5 c.m.)
4.Two(2) photographs showing side and front appearance with chassis and engine numbers certified by the owner.
5.Receipt of Purchase of the motor cycle
6.Customs Documents :Original Packing list ( certified by the Motor Vehicle Examiner), Assessment Notice, Customs 53 Format)
7.Registration Fee is Rs. 4,400.00.

02.Without Customs Documents

For registration of motor cycles without Customs documents, being used by the applicant, following documents also should be submitted in addition to the documents described in 2,3,4 and 5 above.
1.Perfected MTA-2 application Form ( Official stamp should be affixed and signed in relevant places by the agency from which the purchase was made and Chassis and Engine numbers should have been checked and certified by a Motor Vehicle Examiner)
2.A declared affidavit (should contain a statement to the effect that the motor cycle is not subjected to any bond and all responsibilities pertaining should sign on a Rs.25/- stamp on the affidavit which should be certified by a Commissioner of Oaths or a Justice of Peace.
3.A certificate from the Grama Nildhari of the area to the effect that the motor cycle is in applicant’s possession.
4.Registration fee R.4,400.00 + Rs.6,000.00( 300% surcharge)+ R.1,500.00( Fine- default charges)= Rs.11,900.00
